You cannot clean it out. Fog between the panes means the seal on the insulated unit has failed and moisture is now sealed inside the cavity. The repair is a new insulated unit in the same frame, so you keep the window, the sash and the hardware. It is a fraction of the cost of replacing the whole window.
Is it worth replacing every window in a house, or just the failed units?
For most homes, just the failed units. If the frames are sound, replacing only the insulated units that have lost their seal costs a fraction of a full window replacement and gets you the same result. Whole house replacement earns its money when the frames themselves have gone, or when you are changing the windows for other reasons anyway.
