Replace the glass, or replace the door
A sliding patio door is two panels in a frame: one fixed, one that rolls. Each panel is a sash with a large sealed glass unit inside it. When the glass breaks or fogs, the sash, the rollers, the track and the frame are usually all still doing their jobs.
In that case the panel is taken out of the frame, the sash is opened up, the old unit is removed and a new one is set and sealed in its place. The door goes back in the same opening, with the same trim, and slides the way it did before.
Replacing the whole door is a different job. It means removing the frame from the wall, and it is what you need when the frame is bent, the sill has rotted, or the track is worn past the point where new rollers will fix it.

Sliding glass door questions
Can you replace just the glass in a sliding glass door?
Yes, in most doors. The panel is lifted out of the track, the sash is opened, the old glass unit is removed and a new tempered unit is set and sealed in. The frame, track and trim stay in the wall. It only stops making sense when the frame or sill itself has failed.
Does sliding door glass have to be tempered?
Yes. Glass in doors is a hazardous location under the building code, so every sliding and hinged door panel has to be safety glass. Tempered glass also cannot be cut after it is made, which is why the replacement is fabricated to your measurements rather than cut on site.
How long does it take to replace sliding glass door glass?
Two visits. The first measures the panel and checks the frame. The tempered unit is then made to size, and the lead time depends on the glass, so you get the date at the estimate. If the door is broken open, it is secured on the first visit.
Why did my sliding glass door shatter by itself?
Usually an old chip or scratch at the edge of the tempered glass, often from a knock during installation or years of use, that finally lets the stress in the glass release. Less often it is a tiny inclusion in the glass itself. Either way the replacement is the same: a new tempered unit in the existing panel.
Is it worth replacing the glass in an old sliding door?
It is when the door still slides freely, locks, and seals against the frame. If it drags, will not latch, or the sill under it is soft, new glass will not fix those, and the money is better spent on a new door. That is the first thing checked at the estimate.
